Prepare For Your Initial Consultation

Your initial appointment in our office may be one of the most enjoyable dental experiences you have ever had. We take the time with you to explore your concerns, evaluate your current needs, and review your options. Usually, treatment can be started the same day as the consultation to minimize the number of times you have to interrupt your life. A second consultation may be necessary for treatment plans beyond a single tooth or more complex situations. 

To maximize your visit and to make the most efficient use of your time please provide us:

  • Copies of any x-rays taken by previous dentists.
  • A list of medications you are presently taking.
  • If you have dental insurance, bring your dental insurance card. Medical insurance cards do not contain this information. If you are unsure whether you have a dental insurance card, please ask your insurance provider. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian for their first visit.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your previous dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If x-rays do not arrive in time, they can be taken at our office for a nominal fee.
